It has come to my attention that some of the data in my credit report is [inaccurate/incomplete]. In order to correct my report, I am asking that you update your records based on the following and enclosed information.
There is an error regarding the entry of [Creditor]. The entry states [incorrect information]. This information is not correct. It should state [correct information]. I have attached all of the necessary supporting documentation. I would appreciate it if you would update my report with this new information.
